An opportunity has arisen for an experienced Pharmacist to join Primary, Public and Community’s pharmacy service, with a focus on population health across the district and leadership of the pharmacy team.

About the Role

As the Population Health Pharmacist, you will lead and facilitate the Primary Care Support Pharmacist (PCSP) team to enthusiastically and positively influence prescribing behaviours for pharmaceuticals and develop and reinforce best practice prescribing messages.  You will lead the team in the analysis and improvement of prescribing practices across primary care where issues have been identified as contributing to inequities, inappropriate prescribing and sub-optimal health outcomes. This could include, for example, prescribing and medicine related issues arising in age-related residential care, mental health or long term/chronic conditions, nurse-led clinics, or any prescribing issues arising because of contact with hospital services, or as a result of patients’ transition between services.

The role will provide best practice prescribing advice to all primary care prescribers across the MidCentral district, including those prescribers not affiliated with the Central PHO. The aim is to consider the needs and access issues for the wider patient population, with a strong equity focus for priority population groups.

About You

To be considered appropriate for this role, you:

  • Must be a New Zealand registered Pharmacist with a current annual practising certificate and scope of practice which enables the undertaking of the duties of this position.
  • Have strong leadership skills to be able to lead a small team of talented clinicians.
  • Take an enthusiastic approach to your work, and have the ability to lead your team in a manner that fosters collaboration and exceptional business relationships.
  • Be able to build strong business relationships quickly with senior leadership and primary care clinicians.
  • Will have sound analytical and decision-making ability, with the ability to identify trends in population data, including appropriate application of equity focused approaches, to guide best practice prescribing.
  • Will have a strong writing ability to help develop our local best practice guides.
